Philhealth:

o   Claim Form1 (from HR)

o   Philhealth Contributions Table – the hospital requires at least 6 mos contributions

o   MDR 

SSS: 

·         Photocopy of SSS ID with 3 signatures or photocopy of two valid Ids with 3 signatures;

·         Accomplished SSS Maternity Reimbursement (MAT-2) Form;

·         Certified true copy of the child’s birth certificate;

·         Certified true copy of operating room record or surgical memorandum (in case of caesarian delivery);

·         In case of miscarriage or completed abortion:

·         Certified true copy of obstetrical history certified by attending physician and dilatation and curettage (D&C) report for incomplete abortion;

·         Certified true copy of medical report or pregnancy test before and after the miscarriage or completed abortion

Medicard:
